Parents provide the necessary information when completing consent form A and the admission form when the child first enters school. It is important to notify us if any changes in the information occur.

* Appointments –

Denbighshire County Council have adopted appointment forms in co-operation with the health authority. If your child has a medical appointment, a form, available from the school office, must be completed and stamped by the relevant place of appointment.

Medications –

The school understands that at times the pupil we need medicines. To make sure that the correct procedures are followed correctly, there is a form available for parents to complete in reception and to give permission for staff to administer the medication.
